AMOR Wellness is proud to announce a new partnership with Pair Team, a healthcare organization focused on helping people enrolled in Medi-Cal access long-term medical and social support, at no cost.

Through this partnership, our team at the AMOR Wellness Center can now identify and refer clients who may qualify for Pair Team’s Enhanced Care Management (ECM) program. This includes individuals and families enrolled in Medi-Cal that visit AMOR Wellness for emergency food, diapers, public benefits assistance, and more.

If eligible, clients are enrolled in ECM, where they’re matched with a dedicated care team. This team can help coordinate doctor visits, provide support after hospital stays, access additional food, find housing, arrange transportation, connect to mental health and addiction recovery services, and meet other essential needs. ECM is designed to support people facing complex challenges such as chronic health issues, homelessness, and other barriers to care.

By joining the Pair Impact Network, AMOR Wellness is extending its mission to ensure that families in Mendota and surrounding communities gain access to long-term care and stability.
